Rough Shift/ Vibration

I have a 4x4 01 ST with 109,000 miles on it. The transmission seems to be shifting rough. At about 40 mph when shifting into 4th gear there is a vibration in the gas pedal and under the floor then again when going into 5th. Occasionally (not always) the shift into 5th is delayed. The transmission starts to shift then hesitates and finally drops off and almost lurches. No warning lights or codes or anything. I was thinking the valve body gasket, but i not having the "2-3 flair" issue. Any ideas? Thanks

Had the transmission fluid flushed about 12,000 miles ago, fluid level is fine also.

As an inexpensive try, can you disconnect the battery for 10 to 15 minutes? Probably won't help, but it has helped with others and it's cheap and easy.
