high RPM's when shifting gears

I need advise and this was the only place I knew I would get the answers that I am looking for my 05 trac.

When I am stopped at a light and I try to go I have to get the RPM's up to 2500 before the gears switch. It feels like the engine is bogged down. When I go past 60 I have no problem the excelation is ok. Just getting to 60 is the problem. I bought a code reader and there are no codes that have come up. I reset the computer and still the same. what the weird part is that once in while the excelation goes back to normal and the gears shifts very smooth.
 
If it is lugging the motor down in gear, while stoped. It could be the torque convertor, is not unlocking.
 
2500 rpm's is the normal shift point based on my driving habits, maybe up to 3000. Is there a miss in the engine when accelerating from a stop? When was the last time you changed the atx fluid???
